LIABILITY RELEASE AND MEDICAL AUTHORIZATION

I have full and complete understanding: (1) of the physical requirements of participating in the Turkey Trot; (2) that it takes place on campus and public streets; and (3) that medical personnel may not be available. PORTIONS OF THE ROUTE OF THE TURKEY TROT ARE IN A DETERIORATED CONDITION WITH LOOSE GRAVEL, POTHOLES AND CRACKED ASPHALT. In return for being allowed to participate in the Turkey Trot, and with an understanding of the risks of personal injury inherent in participating in the event, I voluntarily RELEASE East Texas Baptist University and its employees, trustees, agents and representatives from ALL claims and lawsuits relating to any loss, illness, damage or injury, including suffering and death, which I may sustain while participating in (or traveling to/from) the Turkey Trot, including but not limited to claims based on negligence, recklessness, strict liability, and fiduciary duty. I represent that I am physically able to participate in the Turkey Trot. If I am injured or become ill during the Turkey Trot, I authorize ETBU to approve emergency medical care for me, including hospitalization, and understand and agree that ETBU has no responsibility for any expense, injury or damage that may arise out of or be in connection with such authorized emergency medical treatment.
